Case Report Form Bookmarks
Metadata → Bookmarks + Annotations → Bookmarks → Add CRF Bookmarks
This process adds a bookmark for every form in your CRF, matched against a list of form titles (and IDs) in your worksheet.
The Metadata tool can quickly add bookmarks to any pdf based Case Report Form (CRF) by quickly finding the form labels and adding a bookmark. This takes literally seconds.
A Case Report Form (CRF) in PDF format.
A List of Form Labels / Names in Microsoft Excel format.
Open Microsoft Excel.
Navigate to the Metadata ribbon.
Locate the Bookmarks + Annotations section of the ribbon.

Click Add CRF Bookmarks to open a prompt.
Select the Case Report Form (CRF) and click OK.

Excel's own range-picker prompt will appear.

Select your Form ID and Form Title columns.

Hold Ctrl and click both columns in your worksheet (or drag across two adjacent ones), Form ID first, Form Title second.
Click OK to continue.

Confirm the output file name.
The default value will be <original file>_Bookmarked.pdf, but you can change it.
If a file with that name already exists, Metadata will automatically pick a safe, non-clashing name rather than overwrite it.
After some time a prompt will appear, Review the prompt, below are two examples of the prompts.

If some titles couldn't be matched, the summary tells you exactly which ones, so you can check those pages by hand rather than assuming full coverage.

A newly bookmarked CRF will be output to the same directory as the one selected at the beginning of the process.
Opening the newly created bookmarked CRF will show that the CRF now contains bookmarks to aid in navigation.
